Embedding Competency Assessment

What the assessment includes
Verification and station readiness
Check cassette identification, tissue count, embedding instructions, paraffin and station temperatures, safe tool use, and tissue carryover prevention.
Tissue orientation and block construction
Document orientation according to grossing instructions and laboratory procedures, mold selection, tissue plane, paraffin handling, and cooling.
Quality checks and block review
Review cassette attachment, tissue recovery, orientation, block defects, corrective action, and re-embedding. Record direct observation, block or section review, and assessment outcome.
Using this template
Review the form against the staff member’s assigned duties and your laboratory’s current procedures. Record the applicable SOP/reference, observed work, review notes, and assessment outcome. Use the document-control fields to identify the version adopted by your laboratory.
